J-U-M-P Referral Form....

Our referral form is for families or organisations who would like to refer a child/family to J-U-M-P for a memory package to be arranged.

All referrals are discussed at our monthly meeting and we try to prioritise and complete as many memories as we can, once a decision is made the family will be contacted by one of our volunteers to arrange with the family the details of the memory

J-U-M-P is a charity run by volunteers who all work normal everyday jobs and devote all the spare time to running the charity. As you can imagine our aim is provide our services to as many families as possible to help us achieve this we can only fund the initial memory.

Should families require any further additions or copies of original memories, unfortunately due to demand and financial budgets this can not be covered by J-U-M-P.

Whilst we will always provide extra additions to memories cost  will incurred by the recipients. This cost will to kept to a minimum.
